Job Description:

">

The Chronic Registered Nurse at DaVita provides empathetic care to patients suffering from end-stage renal failure or chronic kidney disease in an outpatient dialysis setting. The position requires flexibility in scheduling and the ability to float between clinics, ensuring patients receive high-quality care through personalized treatment plans.

">

Responsibilities:

">

• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.

">

• Be part of a team that supports and relies on each other in a positive environment.

">

• Deliver care to patients who are often dealing with multiple comorbidities, requiring unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.

">

• Oversee a group of patients with the support of PCTs to conduct patient observations, measuring stats and machine setup.

">

Requirements:

">

To succeed in this role, you will need:

">

• An Associate's Degree in Nursing (A.D.N) from an accredited school of nursing.

">

• Ability to work flexibly, including mornings, evenings, weekends, and holidays.

">

• A commitment to improving patient health through clinical goal setting and quality improvement initiatives.

">
