We are seeking a skilled and compassionate Skill Builder II to join our team at Meridian Health Services.
About the Role:
As a Skill Builder II, you will provide individual and group skills training and development to assist clients and families in achieving developmental milestones and reducing psychosocial barriers.
Key Responsibilities:
- Conduct interviews, counseling sessions, and provide resolution for clients in situations involving social, emotional, financial, marital, health, or other problems.
- Travel up to 75% to other locations to provide services.
- Develop and implement case management plans, including evaluation, preparation, and implementation.
- Organize and conduct client support groups.
- Maintain working relationships and act as a liaison for clients with medical staff and staff of agencies and institutions.
- Encourage and assist clients toward independent living and a sense of self-esteem.
- Prepare and maintain written case records, reports, and forms, including case follow-up and closing.
- Maintain Meridian Health Services standards, policies, procedures, and regulations in accordance with local, state, and federal laws and with ethical standards of the profession.
- Maintain productivity standards as assigned.
Requirements:
- Bachelor's degree (BA, BS, BSW) in Social Work, Sociology, Psychology, Criminal Justice, Education, Nursing, Recreational Therapy, Rehabilitation, Music Therapy, Art Therapy, Pastoral Counseling, or Child Development.
- One to two years related experience preferred.
- A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
- A commitment to providing individual attention to help clients achieve mental, emotional, and physical well-being.
