Job Title: Per Diem Registered Nurse Perlmutter Cancer Center Huntington Infusion

We are seeking a highly skilled and compassionate Per Diem Registered Nurse to join our team at the Perlmutter Cancer Center Huntington Infusion. As a key member of our clinical team, you will be responsible for providing exceptional patient care and support to individuals receiving infusion therapy in their homes.

Key Responsibilities:
  • Ensure timely and effective communication with patients, families, and healthcare providers to coordinate care and services.
  • Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
  • Provide education and support to patients and families on disease management, treatment options, and self-care techniques.
  • Collaborate with interdisciplinary teams to ensure seamless transitions of care and optimal patient outcomes.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure the highest level of quality and satisfaction.
Requirements:
  • Associate's Degree or equivalent in Nursing.
  • Minimum of five years of clinical experience in the area of specialty.
  • Two years of experience in a supervisory role.
  • RN licensure in the state(s) of desired practice.
  • Acute/Homecare experience within two years and a broad knowledge of case management, governmental home health agency regulations, and Medicare.
  • Excellent organizational, problem-solving, and communication skills.
  • Basic proficiency with computer database entry, Internet, and basic software programs, e.g., Microsoft Office.
