Boston Senior Home Care, a nonprofit organization, is seeking an experienced Registered Nurse to join its team. As a Nurse Care Manager, you will play a vital role in developing care plans for our consumers and connecting them with resources that provide physical, mental, and social support.
The ideal candidate will have a valid Registered Nurse's license from the Commonwealth of Massachusetts and at least one year of clinical experience in a community or long-term care setting. A Bachelor's degree in Nursing is preferred.
We offer a competitive salary of $80,000 per year, commensurate with experience, plus benefits including 27 ETO days annually, insurance (medical, dental, vision, pet), 403(B), T-Pass, and more. Our employees enjoy a healthy work-life balance, with regular full-time hours (35 hours) and no weekend or night shift requirements.
In this role, you will function as part of the interdisciplinary case management team, participating in service planning for Home Care consumers. Responsibilities include clinical assessments to determine medical eligibility for programs funded by the Executive Office of Elder Affairs (EOEA) and the Division of Medical Assistance (DMA). You will also perform in-home nursing assessments to determine personal care needs of Home Care consumers.
