The Center for Human Development (CHD) is seeking an experienced Case Manager to join its team at the Two Rivers Recovery Center in Greenfield.

About the Role:

The Case Manager will be responsible for developing and implementing person-centered treatment plans, facilitating evidenced-based therapeutic groups and individual therapy, assisting with discharge planning, and coordinating community support.

Key Responsibilities:
  • Develop and implement person-centered treatment plans
  • Facilitate evidenced-based therapeutic groups and individual therapy
  • Assist with discharge planning and coordination of community support
  • Collaborate with the Clinical Director and other team members to ensure comprehensive care
Qualifications:

Education: Bachelor's Degree in a related field

Experience: Experience in clinical positions, knowledge of substance use disorders and treatment, and experience facilitating groups

Requirements: One weekend day required
