Job Description:

We are seeking a highly skilled Registered Nurse to join our team as an RN Health Services Coordinator. In this role, you will be responsible for coordinating the healthcare needs of our residents, including developing and implementing care plans, monitoring health outcomes, and ensuring compliance with regulatory standards.

You will work closely with our clinical professionals outside of the agency to provide high-quality care and ensure the well-being of our residents. Your duties will include conducting resident assessments, developing and implementing care plans, and monitoring health outcomes.

The ideal candidate will have a New York State RN License, an Associate's degree in Nursing or higher, and experience working with individuals with intellectual and developmental disabilities. You will also possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

This is a full-time position with a competitive salary range of $70,000 - $90,000 per year, depending on experience. We offer a dynamic and supportive work environment, opportunities for professional growth and development, and a comprehensive benefits package.
