Job Title: Private Duty Nurse LPN

Aveanna Healthcare is seeking a compassionate and skilled Private Duty Nurse LPN to provide high-quality care to our patients in their homes.

Job Summary:

We are looking for a dedicated and experienced LPN to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will be responsible for providing one-on-one care to our patients in their homes, assisting with daily living activities, and administering medications as prescribed by the physician.

Responsibilities:
  • Implement the physician's prescribed plan of care for each patient
  • Conduct patient assessments and coordinate care with the healthcare team
  • Administer prescribed medications, treatments, and therapies as ordered by the physician
  • Document patient care and progress in our electronic health record system
  • Provide education and training to patients and their families on disease management and self-care
  • Collaborate with the healthcare team to develop and implement individualized care plans
Requirements:
  • Active, unencumbered LPN license in the state of Florida
  • Current CPR certification from the American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • 6 months prior hands-on nursing experience preferred but not required
  • Reliable transportation
