Job Summary

Agrace HospiceCare is seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our Homecare team in the Stoughton/Oregon Area. As a CNA, you will provide high-quality patient care and support to individuals in their homes, working closely with our interdisciplinary team to deliver exceptional care.

Key Responsibilities
  • Assist patients with activities of daily living, including bathing, dressing, and grooming
  • Provide personal care, such as toileting and incontinence care
  • Assist with ambulation, changing position, and moving to/from sitting and standing
  • Help with eating and drinking, and perform post-mortem care
  • Obtain vital signs and assist with communication
  • Document cares and changes in patient condition in electronic health records
Requirements
  • High school diploma or equivalent
  • Graduation from an accredited Certified Nursing Assistant program
  • Current, unrestricted Nursing Assistant Certification in state of employment
  • One year of personal care experience, hospice, or home health experience preferred
  • Valid driver's license, auto insurance, and access to a reliable vehicle
