Competitive Compensation: $2700-$2850 weekly gross
Schedule: 8a – 5p, 5 shifts per week
Available Shift: 8 hour days
Location: Worcester, MA
Requirements: At least 1 year of experience in a Medicare-certified home health agency, comfortable with OASIS, valid RN license, and willingness to obtain MA licensure
Job Responsibilities- Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team
- Provide skilled nursing care to patients in their homes, including medication management, wound care, and infusion therapy
- Perform OASIS assessments and documentation accurately and timely to ensure compliance with regulatory requirements
- Educate patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independence
- Monitor patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes
- Provide emotional support and counseling to patients and families coping with chronic illness or end-of-life care
- Competitive hourly pay with overtime opportunities
