BAYADA Home Health Care is seeking a Nurse Care Specialist to provide high-quality care to adult and geriatric patients in their homes. This is an exciting opportunity to join our team of compassionate professionals who are dedicated to delivering exceptional patient care.
Job Description- We are looking for a skilled Registered Nurse with at least one year of prior clinical experience to work as a Nurse Care Specialist in our home health care program.
The successful candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team. A strong background in nursing principles and practices is essential, as well as the ability to think critically and make sound decisions.

Responsibilities- Provide high-quality patient care, following a designated care plan and making home visits to clients in designated geographic territories.
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures, while accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided.
- Monitor clients’ conditions, reporting changes to Clinical or Client Services Manager, and participate in case management and coordination activities.
- A current license as a Registered Nurse in Massachusetts.
- Graduation from an accredited and approved nursing program.
