Job Summary:

As a Registered Nurse at U Mass Memorial Health, you will provide direct nursing care to patients and families, collect and analyze assessment data, and develop individualized patient plans to attain expected outcomes.

Responsibilities:
  • Assure case coordination and provide direct nursing care to patients and families on a 7-day per week basis.
  • Meet patient care productivity standards established by the agency.
  • Collect comprehensive data pertinent to the patient's health or the situation.
  • Analyze assessment data to determine the diagnoses or issues.
  • Identify expected outcomes in a plan individualized to the patient, family, and/or caregivers and the situation.
  • Develop a plan that prescribes strategies and alternatives to attain expected outcomes.
  • Implement the individualized patient plan of care.
  • Coordinate care delivery.
  • Employ strategies to promote health, comfort, and a safe environment.
  • Evaluate progress towards attainment of outcomes.
  • Systematically enhance the quality and effectiveness of his/her nursing practice.
  • Attain knowledge and competency that reflects current nursing practice.
  • Evaluate one's own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules, and regulations.
  • Interact with peers and colleagues, and contribute to their professional development.
  • Collaborate with the patient, family, and others in the conduct of nursing practice.
  • Integrate ethical principles into all areas of practice.
  • Integrate best practices and/or research findings into practice.
  • Consider factors related to safety, effectiveness, and impact on practice in the planning and delivery of nursing services.
  • Provide leadership in the professional practice setting and the profession.

  • Attend staff meetings and in-service programs as required or directed. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices, and reads and responds to emails on a regular basis.
  • Practice cost containment and fiscal responsibility through the efficient use of supplies, equipment, time, etc.
  • Comply with established departmental policies, procedures, and objectives.
  • Attend various meetings, conferences, seminars as required or directed.
  • Demonstrate use of Quality Improvement in daily operations.
  • Comply with all health and safety regulations and requirements.
  • Respect diverse views and approaches, demonstrates Standard of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors.
  • Maintain regular, reliable, and predictable attendance.
  • Performs other similar and related duties as required or directed.
Position Qualifications:
  • License/Certification/Education:
  • Required:
  • Diploma or Associate's Degree in Nursing.
  • Valid and unrestricted licensure as a registered nurse in the Commonwealth of Massachusetts.
  • Valid and unrestricted driver's license recognized by the Commonwealth of Massachusetts and access to a reliable, insured vehicle.
  • Preferred:
  • Bachelor's, Master's, or Doctorate in Nursing.
  • Specialty nursing certification as a home health nurse, hospice and palliative care nurse, or case manager.
  • Completion of the Certified Oasis Specialist certificate.
  • Experience/Skills:
  • Required:
  • 2-5 years of recent (within the past 18 months) medical-surgical experience either in an acute or subacute setting or same experience working in a certified home health agency or licensed hospice.
  • Ability to perform all duties according to American Nurses Association Scope & Standards of Practice for Home Health Nursing professional standards and HAHHH policies and procedures.
  • Ability to use a laptop computer.
  • Ability to proficiently utilize or learn to proficiently utilize MS Office applications, e-mail, Point-of-Care software applications, and other wireless applications.
  • Ability to perform comprehensive physical and psychosocial assessments.
  • Ability to work autonomously.
  • Ability to communicate professionally and effectively with a diverse patient population.
  • Ability to read, write, and speak clearly in English.
  • Preferred:
  • Previous case management or program management experience.
