We are seeking a compassionate and skilled Registered Nurse to join our team as a Nurse Clinician in our Hospice program. As a key member of our interdisciplinary team, you will provide holistic care to terminally ill patients and their families, assessing their needs and developing individualized plans of care.
Responsibilities- Assess patient needs and develop plans of care in collaboration with the IDG
- Provide direct patient care, including medication management and wound care
- Collaborate with other healthcare professionals to ensure continuity of care
- Document patient progress and communicate with families and caregivers
- Participate in quality improvement initiatives to enhance patient outcomes
- Associate's or Bachelor's degree in Nursing
- 1 year of nursing experience, with some pediatric experience preferred
- Current RN licensure in the state of Washington
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
