This role includes an on-call rotation, requiring availability for one night per week and every 3-5 weekends.

Key Responsibilities:
  • Deliver direct oversight and daily assistance to clients primarily within their residences.
  • Establish and uphold a professional and nurturing rapport with all clients, ensuring reliable care while respecting their needs and boundaries.
  • Organize and engage in activities with clients as specified in their care plans.
  • Participate actively in weekly team meetings and group supervision, fulfilling annual in-service training requirements as mandated by state regulations.
  • Maintain a courteous and professional relationship with clients' families and other key supporters.
  • Comply with Stay Graceful's Standard Operating Procedures and relevant state regulations regarding in-home care.
  • Ensure equitable treatment of all clients, colleagues, and family members.
  • Accountable for the quality of care services and completion of essential in-home care tasks, including:

Essential Training:
* Medication Management
* CPR Certification
* First Aid Certification
* Training in life skills and other care methodologies
* Daily support may encompass assisting clients with household chores, meal preparation, grocery shopping, communication with loved ones, and personal hygiene.
* Ensure all necessary documentation and reports are completed by the end of each workday.

  • Demonstrate knowledge of and align with Stay Graceful's Mission Statement, Core Values, and Vision Statement.
  • All In-Home Care Providers are expected to adhere to Stay Graceful's Code of Conduct, Professionalism, and Ethical Standards.

Additional Duties: As assigned.

Education and Experience Requirements:

No formal degree is required; however, relevant experience in Human Services or a related field is strongly preferred. Proficiency in communication via electronic means and familiarity with medical documentation is advantageous. All candidates must possess (or be eligible to obtain) a valid South Dakota driver's license, proof of insurance, and clearance through the Division of Criminal Investigation.

Physical Demands:

Ability to perform the outlined duties, including lifting weights of 10-20 lbs, continuous standing and walking, and frequent verbal communication during shifts. Occasional exposure to hazardous materials may occur.

Note: Candidates with a DUI charge within the last five years or any history of violent crime or felony are ineligible for employment with Stay Graceful.
