- Facilitate care coordination through effective communication with patients, families, and healthcare teams.
- Develop and implement individualized care plans to meet the unique needs of patients.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives to enhance patient satisfaction and clinical outcomes.
- Graduate of an accredited registered nursing program.
- Associate's Degree in Nursing or higher.
- Current Washington Registered Nurse License.
- Current National Provider BLS - American Heart Association certification.
- Related experience in an ambulatory setting, preferably in a medical office.
