Horizon House is a leading provider of psychiatric rehabilitation services, dedicated to helping individuals with mental illness, developmental disabilities, and substance abuse to stabilize their lives and live independently in the community.

We are seeking a skilled Psychiatric Rehabilitation Specialist to join our team in Philadelphia County, PA. As a key member of our Behavioral Health Services Division, you will provide Psychiatric Rehabilitation Services (PSR) in a facility-based and community-based setting, facilitating groups and units to enhance individuals' knowledge and skills in living, learning, working, and socializing.

Responsibilities include:

  • Providing individualized skill development and support, including daily living, socialization, educational/vocational supports, and leisure/recreation activities planning.
  • Maintaining and monitoring an assigned caseload of individuals.
  • Participating in the analysis and identification of service delivery gaps, changes, and problems under the guidance of the supervisor.
  • Consistently meeting productivity expectations as set by administration, department, and funding sources.
  • Participating in the resolution of crisis situations and assisting with identifying individuals and resources that can provide direct support and/or immediate intervention during a crisis situation.

Requirements:

  • Bachelor's degree or Associates degree with 1 year of work experience in mental health direct service, or CPS certification and 1 year of paid or volunteer experience, or High School diploma/GED with 2 years of work experience in Human Services, including 1 year of mental health direct service.
  • Excellent oral and written communication skills, as well as detail-oriented and able to complete a high volume of documentation in a fast-paced environment.
  • Strong computer skills and Internet research abilities.
  • Ability to work independently and as part of a team.
  • Knowledge of mental health system and supports.
  • Valid driver's license with an acceptable driving record.
