Licensed Practical Nurse

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Affirmed Home Care in Connecticut. As an LPN with us, you will be responsible for providing high-quality home health care services to patients in their homes.
Job SummaryAs an LPN, you will work closely with supervising physicians to assess and monitor patients' needs. You will ensure that treatment and care meet the clients' needs, administer medications, and perform other related tasks. This is a Per Diem position, offering flexible working hours.
Key Responsibilities- Assess clients' condition during every visit and track any observations
- Take and record measurements of vital signs
- Administer medication as prescribed by the client's physician
- Dress or redress open wounds and assess progress of healing
- Continuously educate clients, their families, and other caretakers on proper home health care strategies and procedures
- Provide care as needed to keep the client comfortable
- Listen to clients and be prepared to respond to concerns or requests
- Current nursing license in the state
- Physical & TB Testing within 1 year
- Access to reliable transportation
- Ability to work independently and with directions
- Compassionate and friendly demeanor
The estimated salary for this position is around $80,000 per year, based on industry standards and location. We offer competitive pay, flexible working hours, health insurance, and PTO benefits.
