Overview:

The Care Coordinator at Lake Granbury Medical Center is responsible for orchestrating the comprehensive interdisciplinary care plan for patients throughout their hospital journey, from the moment of admission until discharge. This role serves as a crucial link between patients, their families, and healthcare professionals, ensuring that essential medical services are delivered efficiently and effectively.

Key Responsibilities:

  • Facilitate communication among patients, families, and healthcare teams.
  • Utilize clinical expertise and critical thinking to develop and implement care strategies.
  • Monitor patient progress and adjust care plans as necessary.
  • Ensure compliance with healthcare regulations and standards.

Qualifications:

Education:

An Associate's degree in Nursing is required; a Bachelor's degree is preferred.

Experience:

Must possess a valid Registered Nurse license and have a minimum of two years of clinical experience. Familiarity with established concepts, practices, and procedures in nursing is essential.

Licenses and Certifications:

Current nursing license in Texas is mandatory.

Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC) is required.

Certification as a Case Manager must be obtained within three years of employment.
