Position Overview

Full-Time Schedule: 7 Days On, 7 Days Off – Salary Position

Key Responsibilities
  • Conduct initial nursing assessments for new admissions.
  • Develop, coordinate, and update comprehensive care plans while ensuring continuity of care through collaboration with the healthcare team.
  • Regularly communicate patient progress to the clinical manager, physician, and care team.
  • Maintain accurate and timely individualized patient care records.
  • Document changes in pain/symptom management as necessary.
  • Educate families regarding patient conditions, needs, and care strategies.
  • Refer patients to other hospice care specialists as needed, or provide rationale for decisions made.
  • Fulfill on-call responsibilities and provide on-call services to patients and families as assigned.
  • Perform additional duties as required.

Qualifications
  • Current RN license specific to the state of employment.
  • Minimum of one year of nursing experience.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
