Position Overview

Join our team to provide compassionate care when it matters most.

As a Hospice Registered Nurse / RN Case Manager, your responsibilities will include:

  • Directly reporting to the Administrator, Executive Director, or Patient Care Manager Senior while maintaining overall control and professional oversight of each patient’s care.
  • Serving as the primary communication link between the physician, patient/family, and hospice team.
  • Delivering direct care services to hospice patients and providing supportive assistance to their families, ensuring a manageable caseload and fostering a trusting relationship.
  • Empowering caregivers by offering education on the care required for their loved ones.
  • Collaborating with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to promote comfort, peace, and dignity.
  • Assessing patient and family needs and contributing to the development and implementation of personalized care plans.
  • Maintaining accurate clinical records for all assigned patients.
  • Engaging in company quality assessment and performance improvement initiatives.
Candidate Profile

Qualifications for the Hospice Registered Nurse / RN Case Manager role:

  • A compassionate heart, demonstrating kindness and empathy towards patients and their families.
  • Comfortable providing care in various home settings.
  • Ability to remain flexible and composed while supporting others during challenging times.
  • Current RN license in the state of employment (or eligibility to obtain).
  • Preferred experience of 1+ years in hospice, home health, ICU, geriatric, or oncology nursing.
  • Additional experience in hospital settings such as Med Surg, PACU, Float pool, ER, critical care, or telemetry is advantageous.
  • Possession of a valid driver’s license and automobile insurance.
  • Current CPR certification.
