Responsibilities:

- Documents patient care in a concise, accurate, and complete manner.
- Completes population-specific mandatory education competencies.

- Works collaboratively with patients and families of diverse backgrounds to develop a plan of care.
- Is tolerant of differences posed by patients and families while cognizant of personal biases.
- Practices evidence-based care and utilizes reliable resources to support practice change.
- Utilizes effective communication and collaboration skills to work with the interdisciplinary team.
- Manages time effectively to accomplish assigned tasks and meets expectations for punctuality and attendance.
- Contributes to a healthy work environment by consistently exhibiting professional behaviors and addressing interpersonal conflicts with the right person, at the right time, in the right location.
Requirements:
- Valid driver's license with reliable transportation and agency required auto liability insurance.
- BLS for Healthcare Providers is required to be obtained prior to independent visits.
- 1 year/years of clinical experience.
Working Conditions:
- Lifting, moving and loading 30 to 50 pounds.
