Job Summary

We are seeking a highly skilled Occupational Therapist Assistant to join our team at Alternate Solutions Health Network LLC. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and functional ability.

Key Responsibilities
  • Plan of Care: Develop and implement individualized plans of care, in collaboration with the attending physician and occupational therapist, to address patients' functional needs and goals.
  • Minimize Patient Risk: Assess patients' functional abilities and provide interventions to prevent falls, injuries, and other adverse events.
  • Family Educator/Advocate: Educate patients and their families on disease management, proper use of equipment, and strategies for maintaining independence.
  • Policies and Procedures: Maintain accurate and timely documentation, adhering to agency protocols and regulatory requirements.
  • Rules and Regulations: Ensure compliance with agency policies, procedures, and regulatory guidelines, including those related to patient confidentiality and HIPAA.
  • Operations: Participate in in-service training and mandatory meetings to stay current on agency policies, procedures, and best practices.
Requirements
  • Education and Licensure: Current licensure as an Occupational Therapist Assistant (OTA) in the state of employment.
  • Experience: Minimum one year of experience as an OTA in an acute care setting, with home care experience preferred.
  • Skills and Abilities: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, physicians, and other healthcare professionals.
