Job Summary:
IntelyCare is seeking a Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN with IntelyCare, you will have the opportunity to work in post-acute facilities in the area, providing high-quality patient care and making a meaningful difference in the lives of our residents.
Responsibilities:
- Provide direct patient care, including administering medications, monitoring vital signs, and conducting intakes and documenting health concerns and medical history
- Collaborate with healthcare teams to develop and implement resident care plans
- Conduct regular assessments of resident wellbeing, including physical and mental health
- Administer and properly verify medications, monitoring their effects and reporting any adverse reactions
- Review and update resident charts and medical records as needed
- Participate in IV set up, immunizations, and phlebotomy procedures
- Assist with the orientation and training of medical staff
Requirements:
- Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
- Completion of approved Licensed Practical Nurse (LPN) educational program
- High school graduation, GED, or equivalent
- At least 6 months of experience (LPN or similar role)
- Ability to lift a minimum of 50 lbs
