Job Title: Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) Job Specialty: Long Term Care / Skilled Nursing Facility Job Duration: 15 weeks Shift: 8-hour evenings, 2:45 PM - 11:00 PM, every other weekend Guaranteed Hours: 38 hours per week Experience: Minimum 1 year License: Active LPN/LVN License Certifications: Basic Life Support (BLS) Must-Have: Prior experience with PointClickCare (PCC), wound measurement experience Job Description: - Provide care to residents in a long-term care/skilled nursing facility, focusing on medication and treatment administration.- Perform tasks such as blood glucose monitoring, insulin administration, and IV medication management (including antibiotics, Lasix, and Bumex).- Conduct tube feedings, wound care and measurement, lab draws, specimen collection, foley catheter care, and tracheostomy care.- Assist patients with activities of daily living (ADL) and perform assessment charting and incident reporting.- Monitor vital signs and administer oxygen therapy as needed.- Participate in care plan meetings, primarily for short-term stay residents.- Work evenings under the supervision of an RN Supervisor, collaborating with charge nurses and medication/treatment nurses.- Ensure professional communication with the interdisciplinary team, including physical and occupational therapists, speech-language pathologists, case managers, social workers, and dieticians.- Adhere to the dress code, wearing clean and professional scrubs (any color, excluding camouflage).- Participate in a 3-day orientation specific to the unit/department.- Follow scheduling requirements, including working on holidays if they coincide with scheduled shifts.- Maintain flexibility to float to different areas within the facility if necessary.
