We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire.
Job SummaryThe Registered Nurse Case Manager will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.
Responsibilities- Manage a caseload of patients, providing direct care and coordinating services as needed.
- Conduct thorough assessments of patients' needs and develop individualized care plans.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Provide education and support to patients and their families on disease management and self-care.
- Document patient progress and outcomes in a timely and accurate manner.
- Valid nursing license in the state of New Hampshire.
- 2+ years of experience in skilled care, preferably with a focus on case management.
- 1+ year of experience in nursing supervision or leadership.
- Recent wound care experience and knowledge of wound management principles.
- Ability to lift 50 pounds with no restrictions.
- Current CPR certification.
- Reliable transportation.
