LPN Team Lead Job Description

Innova Solutions is seeking a highly skilled LPN Team Lead to join our team. As an LPN Team Lead, you will play a crucial role in providing exceptional patient care and leading a team of medical professionals.

Key Responsibilities:
  • Provide services to meet the diverse needs of clients through coordination with the medical team.
  • Supervise medical and day-to-day operations of substance use disorders/mental health services.
  • Ensure adherence to state board of nursing and state nurse practice act requirements and other governing agency regulations.
  • Maintain nursing staff by interviewing, training, supervising, orienting, and training nurses.
  • Promote patients' independence by establishing patient care goals, teaching and counseling patients and their families individually and/or in groups, and reinforcing their understanding of disease, medications, and self-care skills.
  • Protect patients and employees by developing and interpreting infection-control policies and protocols, enforcing medication administration, storage procedures, and controlled substance regulations.
  • Perform Quality Assurance activities.
  • Provide services according to agency policies, procedures, program guidelines, and governmental regulations.
  • Monitor provision of services when assigned.
  • Assure program record documentation compliance with agency and regulatory standards.
Requirements:
  • Must be a valid and active LPN, licensed with NYS.
  • One to three years of clinical experience appropriate to program preferred.
  • Minimum three years clinical experience in an appropriate program setting.
  • An equivalent combination of training and experience may be substituted for educational requirements.
  • Must possess a valid NYS Driver's License.
  • Excellent communication skills.
