Job Title: Registered Nurse (RN) Home Hospital Nurse Navigator

Mass General Brigham is seeking a skilled Registered Nurse (RN) to fill the role of Home Hospital Nurse Navigator in our Chelsea, Massachusetts location.

Job Summary:

The Home Hospital Nurse Navigator is responsible for identifying eligible patients for the MGB Home Hospital program and conducting clinical and social eligibility and safety screening. This role requires strong communication and collaboration skills to work with the Home Hospital Admitting provider, Case Manager, and Administrative Coordinator to facilitate transfers to MGB Home Hospital.

Responsibilities:
  • Attend daily huddles with the Home Hospital team to gather clinical information and identify patients based on catchment area and plan of care.
  • Interact with the Emergency Department interdisciplinary team and ED Care Facilitators to assist patient identification and transfer to MGB Home Hospital.
  • Collaborate closely with the MGB Home Hospital Admitting Provider to notify the Home Hospital Case Manager and obtain consent from patients for transfer.
  • Communicate and collaborate with the Administrative Coordinator and Administrative Manager regarding patient transfers.
  • Utilize analytical and critical thinking in decision making and judgment to align with organizational objectives.
Requirements:
  • Current RN licensure in the state of Massachusetts.
  • Minimum 3 years of experience in emergency department, home hospital, or inpatient settings.
  • Varied experience preferred.

This role is based in a hospital environment, working in an emergency department setting.
