Key Responsibilities
  • Assessment and Intervention: Conduct thorough psychosocial assessments to identify the needs of patients and their families, providing individualized support and interventions to promote optimal outcomes.
  • Crisis Intervention: Develop and implement crisis intervention plans to address complex situations and ensure the well-being of patients and their loved ones.
  • Community Referrals: Facilitate referrals to community resources, ensuring seamless access to necessary services and support.
  • Nursing Home/Assisted Living Facility Transfers: Coordinate transfers to optimize patient care and meet their unique needs.
  • Bereavement Support: Provide empathetic support to patients and families coping with bereavement, helping them navigate this difficult time.
Requirements
  • MSW from a CSWE-accredited school
  • Licensed or licensed eligible as a Clinical Social Worker in Florida
  • Valid driving license and good driving record
  • One year of healthcare experience
  • Social work or counseling background, knowledge of community resources
  • Strong clinical and assessment skills, including crisis intervention and teamwork
  • Excellent organization, time-management, and self-awareness skills
  • Customer-service oriented, effective communication, and relationship-building skills
  • Ability to provide care for pediatric, adolescent, adult, and geriatric patients
