Responsibilities- Develop, coordinate, and maintain therapeutic services and activities alongside an interdisciplinary team.
- Assess patients' physical abilities and create personalized treatment plans to address their needs.
- Provide one-on-one therapy sessions to patients in their homes, focusing on improving mobility, strength, and function.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Document patient progress and adjust treatment plans as needed.
- Bachelor's Degree in Physical Therapy.
- Washington Physical Therapist License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Master's Degree in Physical Therapy or Ph.D. in Physical Therapy.
- 1 year of experience in a therapist capacity in a hospital, skilled nursing facility, or other related medical field.
- Home Health experience.
