About the Role

Telecare's Kaiser Intensive Community Treatment (ICT) program is seeking a skilled Case Manager II to join our team. As a primary support and case manager, you will work directly with individuals with mental illness and other co-occurring disorders to help them live independently in their chosen community.

Under the supervision of our experienced team, you will assist clients in achieving their goals and improving their overall well-being. Your dedication and compassion will make a significant difference in the lives of our clients.

Key Responsibilities:

  • Provide direct support and case management services to clients with mental illness and other co-occurring disorders
  • Assist clients in developing and implementing individualized treatment plans
  • Collaborate with our multidisciplinary team to ensure comprehensive care and support
  • Provide crisis intervention and support as needed

Requirements:

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
