Case Manager II

Job Summary:
We are seeking a highly motivated and compassionate Case Manager II to join our team at LifeLong Medical Care. As a key member of our interdisciplinary team, you will provide services for patients with complex care needs, conducting outreach, engagement, and psychosocial service assessments, and developing patient-centered care plans.
Responsibilities:
- Conduct patient outreach and engagement to identify and assess patients' needs and strengths.
- Develop and implement patient-centered care plans in collaboration with patients, caregivers, and healthcare providers.
- Provide referrals for internal and external resources, and facilitate access to care and services.
- Maintain accurate and up-to-date records of patient interactions and services provided.
- Collaborate with patients, caregivers, and healthcare providers to ensure seamless care coordination and transitions.
- Provide education and support to patients and caregivers on health and wellness topics, including harm reduction and disease risk-mitigation strategies.
- Advocate on behalf of patients to address social determinants of health and access to resources and services.
Requirements:
- High School diploma or equivalent required; Bachelor's degree in Social Work or related field preferred.
- At least two years of experience in a community-based healthcare or social work setting, with a focus on case management and care coordination.
- Strong organizational, communication, and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care and social justice.
