Pediatric Nurse Case Manager Job Description

We are seeking a skilled Pediatric Nurse Case Manager to join our team at The Kidz Club PPEC. As a Pediatric Nurse Case Manager, you will be responsible for managing the care of pediatric patients, ensuring their individual needs are met and their care plans are implemented effectively.

Key Responsibilities:

  • Manage the care of pediatric patients, including developing and implementing individualized care plans
  • Facilitate referrals to primary providers, specialists, healthcare facilities, and community resources
  • Provide direct client care, including a holistic approach to the care of children and their families
  • Educate clients and families regarding immediate care needs and preventative behaviors

Requirements:

  • Graduate of an accredited school of nursing with current RN licensure eligible in the state of Kentucky
  • Recent graduate with a provisional RN license
  • Basic personal computer skills and comfort with learning electronic charting systems

Preferred Qualifications:

  • At least six months of current experience as a practicing RN with pediatric experience
  • Current negative Tuberculosis skin test (PPD) required
  • Current CPR (Basic Life Support) is required
