Job Summary:

Under the direction of the Registered Nurse, the LPN will provide care to stable patient populations with predictable outcomes. The ideal candidate will have strong communication skills, be able to work effectively in a team environment, and possess a commitment to delivering excellent patient care.

Responsibilities:
  • Conduct patient-centered histories and obtain initial physical data, including vital signs and weight.
  • Incorporate available data and nursing knowledge to identify patient care needs.
  • Utilize evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communications.
  • Administer medications and immunizations according to hospital policy and procedure.
  • Collect specimens and perform routine laboratory tests.
  • Monitor and maintain lab equipment and supplies.
  • Provide age-appropriate care in a compassionate and culturally sensitive manner.
  • Participate in the development and implementation of the interdisciplinary plan of care.
  • Document patient information in accordance with CHA standards.
Requirements:
  • Graduation from an accredited licensed practical school of nursing.
  • MA LPN licensure required.
  • Maintains current Basic Life Support certification.
  • Knowledge of a second language preferred.
