Intellectual Disabilities Support Coordinator

- Conduct initial and annual assessments of individuals residing in Residential Waiver Group homes
- Develop and implement Individual Support Plans to meet the needs of each individual
- Train staff on plan implementation and ensure effective communication with those involved
- Travel within a designated catchment area to conduct assessments and provide support services
A Bachelor's degree in Human Services and at least one year of paid experience working with individuals who have intellectual disabilities are required for this role. A competitive salary range of $65,000 - $85,000 per annum, depending on qualifications and experience, is offered.
