About the Role

Gentiva Hospice is seeking a skilled PRN Hospice Nurse Practitioner to join our team. As a key member of our hospice team, you will be responsible for performing Face-to-Face (F2F) encounters and gathering clinical information to support the hospice diagnosis and prognosis.

Key Responsibilities
  • Perform F2F Encounters: Conduct physical assessments for F2F encounters within regulatory time frames established by CMS.
  • Provide Clinical Findings: Communicate clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists to the physician responsible for certifying or recertifying patient eligibility.
  • Make Recommendations: Make appropriate recommendations to the Interdisciplinary Group (IDG) based on findings of the F2F assessment.
  • Notify Hospice Agency: Notify the hospice agency when unable to complete a F2F visit according to schedule.
Requirements
  • Master's or Higher Degree in Nursing: Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
  • Current State License: Current state license as a registered nurse and current state license or certificate as an advanced practice nurse.
  • National Certification: Current national certification as a Nurse Practitioner (ANCC or AANP).
  • Collaborative Agreement: Must have a collaborative agreement in place.
