Key Responsibilities
  • Coordinate services with case managers and parent/guardians to ensure the provision of high-quality services.
  • Document and present each member's goals and objectives in accordance with ISP regulations.
  • Oversee the screening and prioritization of referrals, and implement behavior management strategies as needed.
  • Investigate incidents and ensure the provision of high-quality services.
Requirements
  • At least 1-2 years of prior case management or care management experience required.
  • Ability to use computers, including Microsoft Word, data entry, email, and internet essential.
  • Experience in Credible preferred.
  • Must be organized, detail-oriented, have solid time management skills, and be able to work in a fast-paced multi-tasking environment.
  • Demonstrated ability to deliver a high level of customer service/satisfaction with both internal and external customers.
  • Excellent communication, interpersonal, and listening skills.
  • Must have a valid driver's license, good driving record, use of vehicle, and willingness to transport participants in personal and/or agency vehicle.
