Job OverviewThe Licensed Practical/Vocational Nurse (LVN/LPN) plays a vital role in providing high-quality nursing care and education to clients and families. As an Assistant to the physician and Registered Professional Nurse, the LVN/LPN will work under the supervision of the RN to ensure seamless care delivery.
Responsibilities- Participate in the planning and coordination of total client care in conjunction with the RN, agency policies, and the physician's Plan of Care.
- Follow the nursing care plan for each assigned client, observing and evaluating their condition to apply knowledge of nursing skills.
- Accurately report and record the client's condition and care, including signs and symptoms that may indicate a change.
- Prepare clinical/progress notes on the day of the visit and incorporate them into the clinical record weekly, summarizing in cooperation with the RN supervisor.
- Assist clients with activities of daily living while teaching self-care techniques, providing a safe environment, and promoting physical and emotional well-being.
- Assist the physician and Registered Nurse in complex nursing situations and perform specialized procedures as qualified.
- Prepare equipment and materials for treatments, observing aseptic technique as required.
- Participate in case conferences, inservices, and other programs as needed, performing within the limits of preparation and experience.
- Confirm weekly scheduling of visits with the nurse to coordinate necessary visits with other personnel.
- Perform treatments as assigned per physician's plan of care, adhering to qualifications and experience.
The LVN/LPN will report directly to the Director of Nursing Services.
Special Requirements- A valid driver's license and reliable transportation are required.
- Effective verbal and written communication skills.
- Ability to physically perform the duties required by the position.
- Ability to travel to clients' residences.
