Job Title: Hospice RN Case Manager

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and coordinate services to ensure the best possible outcomes for our patients and their families.

Job Summary:

The primary functions of this role include administering skilled nursing care, coordinating care with the interdisciplinary team, patients, and their families, and referring to other services as needed. You will assume responsibility for the coordination of care and ensure that patients receive the highest level of care possible.

Responsibilities:
  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of patients.
  • Develop, initiate, and revise the plan of care as necessary to ensure quality and continuity of care.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Refer to other services as needed.
  • Plan for the discharge of patients from services.
  • Furnish those services requiring substantial and specialized nursing skill.
  • Counsel patients and their families in meeting nursing and related needs.
  • Use infection control measures that protect both staff and patients.
  • Coordinate services.
  • Inform physicians and other personnel of changes in patients' conditions and needs.
  • Monitor assigned cases to ensure compliance with requirements of third-party payors.
  • Prepare clinical and progress notes.
  • Complete appropriate documentation in a timely manner.
  • Demonstrate commitment and professional growth by participating in in-service programs and maintaining or improving competency.
  • Supervise, teach, and provide clinical direction to other nursing personnel.
Requirements:
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State license as a Registered Nurse.
  • Current Driver's License.
  • One year of experience as a Registered Nurse in a clinical care setting required. Home health experience preferred.
  • Nursing skills as defined by generally accepted standards of practice.
  • Good interpersonal skills.
  • Proof of current CPR.
  • Reliable transportation and valid and current driver's license and auto insurance.
