Key Responsibilities- Conduct Comprehensive Assessments: Perform thorough assessments in patients' homes or healthcare facilities to determine their needs and eligibility for our Community Care Program.
- Develop Care Plans: Create personalized care plans that meet the unique needs of each patient and their caregivers.
- Coordinate Services: Authorize and coordinate referrals to other benefits and services available to patients, ensuring they receive the support they need.
- Advocate for Patients: Act as a patient advocate, intervening and following up in a timely manner to ensure services remain in place and are appropriate to their changing needs.
- Document and Follow Up: Accurately document and complete follow-up on critical events, updating care plans and completing redeterminations as needed.
- Meet Timelines: Meet established timelines for assessments and critical event follow-up, ensuring timely and efficient service delivery.
- Participate in Staff Meetings: Participate in departmental and all-agency staff meetings, offering constructive input and contributing to a collaborative work environment.
- Education and Experience: A minimum of 1 year of experience in needs assessment and care planning, with a degree in social science, social work, or a related field.
- Certifications and Licenses: Approved CPR or BLS certification, IL Department on Aging Care Coordinator Certification, and a valid driver's license with reliable transportation.
- Skills and Abilities: Excellent customer service skills, effective verbal and written communication, and the ability to work in a fast-paced environment.
