About the Role:
Sugar Maple Health is seeking a skilled Physical Therapist to join our team in providing high-quality care to our geriatric patients in the comfort of their own homes or senior living facilities.
Key Responsibilities:
- Deliver personalized physical therapy services to patients with various geriatric conditions, focusing on improving mobility, strength, and overall well-being.
- Conduct thorough assessments, develop, and implement individualized treatment plans to meet patients' unique needs.
- Collaborate with healthcare professionals, caregivers, and family members to ensure seamless care coordination.
- Provide education and guidance to patients and their support networks on proper care techniques, exercise routines, and disease management.
- Document patient progress, maintain accurate records, and communicate effectively with the healthcare team.
What We Offer:
- A competitive salary range starting at $80,000 per year, based on experience.

- Ultimate schedule flexibility, allowing you to work when you want, for as many patients as you want to see (minimum 12.5 hours of direct patient care and 3 weekdays of availability per week).

Requirements:
- Current licensure as a Physical Therapist in the state of New Jersey.
