Per Diem Nurse Practitioner/Physician Assistant

Job Summary
Optum Home and Community Care is seeking a skilled Nurse Practitioner/Physician Assistant to provide high-quality care to our members in a per diem capacity. As a member of our Senior Community Care team, you will work with a collaborative team to deliver cost-effective, quality care to patients in their homes, nursing homes, and assisted living facilities.
Responsibilities
- Primary Care Delivery
- Deliver comprehensive assessments and develop personalized care plans for assigned members
- Manage medical and behavioral conditions, including chronic and acute care
- Collaborate with physicians and specialty providers to ensure seamless care coordination
- Document findings and diagnoses accurately and efficiently
- Care Coordination
- Understand payer and plan benefits, as well as CCM associate policies and procedures
- Assess medical necessity and effectiveness of ancillary services
- Coordinate care transitions and monitor member needs
- Review orders and interventions for appropriateness and response to treatment
- Program Enhancement
- Communicate effectively with internal and external stakeholders, including physicians, members, and facility staff
- Partner with key stakeholders to promote the CCM program and maintain membership growth
- Develop and maintain positive customer relationships
Requirements
- Certified Nurse Practitioner through a national board
- Active and unrestricted license in the state of practice
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire
- Access to reliable transportation and ability to travel to client and/or patient sites
- Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions
- Availability to work 24 hours per month, with expectations of 16 hours during off-hours
