We are seeking a highly skilled and experienced Registered Nurse to lead our Care Management team. As a Care Management Team Lead, you will be responsible for guiding the daily operations of the care management department, planning and organizing all care management functions, and leading initiatives to improve processes and meet departmental goals.
Key Responsibilities
* Guides care management staff in all daily operational performance matters
* Uses all available financial and performance management information to monitor and measure the performance of the department and team members
* Serves as a model in customer service matters and educates care management staff in delivering outstanding customer service
* Strategizes with leadership to achieve departmental and organizational goals
* Adapts behavior to the specific patient population and performs other related duties as required
Requirements
* Masters degree in Social Work (MSW) or current LPN or RN License in the state of practice
* 3 years of utilization and/or case management experience with either a LPN license, RN license, or MSW
* Preferred: Supervisor or leadership experience and current Licensed Practical Nurse (LPN) license in state of practice or Current Registered Nurse (RN) license in state of practice
