Job Summary

We are seeking a dedicated and compassionate Registered Nurse to join our team as an AFC Nurse. As a member of our nursing and care management team, you will provide nursing services to facilitate the screening of new foster homes and participants, assist with matching and placement services, and maintain services to participants as needed.

Key Responsibilities
  • Provide oversight to caregivers in nursing care of participant, coordinate participant care with local home health agencies, educate, supervise, and evaluate caregivers in care of program participants, and participate in ongoing supervision of participants in homes via regular outreach visits.
  • Screen and assess referrals for services in conjunction with case management staff, obtain approval for referrals assessed to be appropriate, and operate within the scope of professional nursing ethics and practice.
  • Develop a care plan for participants, review plan of care with the participant and caregiver, provide ongoing evaluation and revision semi-annually or on an as needed basis, and assist in the development, implementation, and evaluation of home caregiver training sessions.
  • Record and maintain accurate and timely case records, prepare and submit detailed documentation for Adult Foster Care consideration, meet with guardians, doctors, and relevant parties as needed, and prepare and submit individual service plan (ISP) goals and assessments for individuals funded by the Department of Developmental Services (DDS).
  • Provide on-call coverage on nights and weekends as scheduled, maintain professional growth and development through approved seminars, workshops, and professional affiliations, and provide consultation to management with respect to health care policies for the program.
  • Serve as a mandated reporter of suspected cases of abuse and/or neglect to the Disabled Persons Protection Commission (DPPC) and perform additional duties as assigned.
Requirements
  • Registered Nurse licensed by the Massachusetts Board of Registration in Nursing.
  • Minimum 2 years prior nursing experience in the direct care of the elderly or disabled adults.
  • Must be compassionate and patient.
Working Conditions/Physical Demands

Frequent sitting, standing, and walking. Ability to sit for prolonged periods of time in a car traveling from location to location.
