Key Responsibilities:

As a Social Worker, you will:

• Evaluate the psychosocial status of patients in relation to their health conditions and living environments, and relay findings to the nursing staff.
• Conduct social assessments and develop intervention strategies based on your evaluations.
• Maintain accurate clinical records for all patients referred to social work services.
• Document clinical and progress notes effectively.
• Provide information and referral services to patients and their families regarding their physical and environmental needs.
• Act as a liaison between patients, families, and community resources.
• Foster collaborative relationships with other healthcare personnel to enhance patient care.
• Develop and maintain partnerships with public and private agencies for resource availability.
• Contribute to the overall care plan and participate in case conferences as necessary.
• Assist healthcare providers in recognizing significant social and emotional factors impacting health.
• Engage in discharge planning processes.


Qualifications:
• Master’s degree in social work from an accredited program recognized by the Council on Social Work Education, or a doctoral degree in social work.
• At least one year of experience in a healthcare setting, with a preference for home health care experience.
• Strong verbal and written communication skills, along with excellent organizational abilities.
• Current CPR certification is required.
• Must possess reliable transportation, a valid driver’s license, and appropriate automobile insurance.
