**Job Title:** Community Based Care Clinician
**Job Summary:**
Community Care Alliance is seeking a skilled Clinician to join our multidisciplinary team. The successful candidate will provide individual and family treatment for children and youth at risk of higher-level care or out-of-home placement.
**Key Responsibilities:**
- Provide intensive individual and family counseling utilizing various modalities to address complex trauma, social determinants of health, and social challenges.
- Collaborate with a multidisciplinary team and other agencies to ensure comprehensive care for clients and families.
- Function as the clinical lead on shared cases, providing direction on need and interventions.
- Develop and implement treatment plans tailored to the unique needs of each client and family.
**Requirements:**
- Master's degree in social work, counseling, psychology, or a related human services discipline.
- License eligible or independently licensed. Supervision for independent licensure provided.
- Driver's license required.
- Bi-lingual candidates preferred.
