Job Summary
AdvisaCare is seeking a compassionate and experienced Medical Social Worker to join our Home Health and Hospice team in Northern Michigan. As a Medical Social Worker, you will play a vital role in enhancing the lives of patients with life-limiting illnesses and their families.
Key Responsibilities
- Conduct initial psychosocial assessments and develop goal-directed care plans in collaboration with the interdisciplinary team.
- Provide emotional, spiritual, and bereavement support to patients and families, and facilitate referrals to community agencies as needed.
- Document all patient interactions, assessments, and care plans in a timely and accurate manner.
- Participate in IDT conferences and contribute to the development of comprehensive care plans.
- Provide crisis intervention and individual or family counseling as required.
- Develop and maintain relationships with community agencies and services to promote interagency cooperation and facilitate referrals.
- Participate in the Bereavement Team and adhere to all Hospice policies.
Requirements
- Master of Social Work (MSW) degree from a Council on Social Work Education (CSWE) accredited school.
- One year of experience in Home Health Care, with Hospice experience preferred.
- Valid Driver's License, car insurance, and reliable transportation.
