Key Responsibilities:- Coordinate and support patient-centered mental health care within the parameters of the Health Center's Patient Centered Medical Home Certification.
- Evaluate and gather information from patients regarding mental health and substance abuse disorders.
- Use measurement-based tools to track patients and gather information about treatment side effects or complications.
- Support psychotropic medication management and facilitate in-clinic or outside referrals to evidence-based psychosocial treatments.
- Participate in regularly scheduled caseload consultation with the Psychiatric Consultant and facilitate communication regarding treatment recommendations to the patient's PCP.
- Masters in Social Work or Clinical Mental Health Counseling required.
- Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC), or Licensed Addiction Counselor (LAC) in the state of Montana.
- Valid Driver's License issued by the State of Montana.
Work indoors in a climate-controlled environment 95% of the time. OSHA Exposure Category #1.
This is a full-time, exempt position with a 40-hour workweek. Monday - Friday, 1.0 FTE - School Contract.
