We are seeking a highly skilled Advanced Practice Clinician to join our team at Optum. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and working closely with our medical staff to ensure the best possible outcomes for our patients.
Key Responsibilities- Patient Care: Provide comprehensive patient assessments, develop and implement individualized care plans, and coordinate with other healthcare professionals to ensure seamless care delivery.
- Collaboration: Work closely with our medical staff, including physicians and other advanced practice clinicians, to ensure effective communication and coordination of patient care.
- Education and Support: Educate patients and their families on disease management, health promotion, and wellness strategies, and provide emotional support and guidance as needed.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes, reduce readmissions, and improve overall healthcare delivery.
- Documentation: Maintain accurate and timely documentation of patient interactions, assessments, and care plans in accordance with regulatory requirements and Optum's policies and procedures.
- Education: Completion of a Nurse Practitioner or Physician Assistant accredited program.
- License: Current state NP/PA license.
- Experience: 1+ years of clinical experience as a NP or PA.
- Skills: Strong communication and interpersonal skills, ability to work effectively in a team environment, and proficiency in electronic medical records and other healthcare software systems.
- Experience: 2+ years of clinical experience.
- Knowledge: Familiarity with long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
