Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Hope At Home Health Care.
As a LPN, you will provide professional nursing care to patients in their homes, working closely with our team to deliver high-quality care.
Key Responsibilities:
* Provide skilled nursing care to patients in accordance with their individual needs and physician orders
* Maintain accurate and detailed clinical and progress notes
* Assist the Registered Nurse or physician in performing specialized procedures and duties
* Participate in established Agency committees and attend mandatory staff meetings and training programs
Requirements:
* Graduate of an accredited School of Nursing and current licensure in the State of practice
* One year of recent experience in adult patient home care
* Excellent observational and assessment skills, good verbal and written skills, and a valid driver's license
