About the Role:
We are seeking a skilled RN to join our team as a Clinical Transition Specialist. In this role, you will play a critical part in ensuring the seamless transition of patients from the hospital to their next level of care.
Key Responsibilities:
- Supports the Progression of Care: Collaborate with the healthcare team to ensure timely progression of care for complex patients, identifying and eliminating potential delays or barriers.
- Clinical Transition Coordination: Coordinate the clinical transition of patients in the hospital and Emergency Department, working closely with the interdisciplinary team to plan anticipated transfers or discharges.
- Patient and Family Support: Provide information and support to patients and families regarding palliative care and advanced care planning, ensuring they are prepared for their next level of care.
- Readmission Prevention: Participate in care system processes to prevent potentially preventable readmissions, utilizing tools and technology to identify and intervene with patients at risk.
- Length of Stay Management: Collaborate with the healthcare team to promote appropriate length of stay, facilitating timely referrals and transfers of information.
Requirements:
- Education: Bachelor's degree in nursing
- Experience: 2-5 years of nursing experience, with a minimum of 2 years in an acute care setting
- Certifications: Licensed Registered Nurse - MN Board of Nursing or WI Dept of Safety & Professional Services, Case/Care Management Certification within 2 years of hire
