As a Hospice RN / RN Case Manager, your responsibilities will include:
- Reporting directly to the Administrator, Executive Director, or Patient Care Manager, while maintaining primary control and professional oversight of each patient.
- Serving as the main point of contact between the physician, patient/family, and hospice team.
- Delivering direct patient care services to hospice patients and providing supportive care to their families, ensuring a trusting and compassionate relationship.
- Empowering caregivers by educating them on how to care for their loved ones.
- Collaborating with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity for patients.
- Assessing patient and family needs and assisting in the development and implementation of individualized care plans.
- Maintaining all assigned patient clinical records accurately.
- Participating in company quality assessment and performance improvement initiatives.
Requirements for Hospice Registered Nurse / RN Case Manager:
- A compassionate heart, demonstrating kindness and empathy towards patients and their families.
- Comfortable providing care in various settings as determined by the patient's needs.
- At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Additional experience in hospital settings such as Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is advantageous.
